Here is the level of cover you will receive with 'we plan' Level 5. Remember you are fully covered for neuro-surgery, cancer, chemotherapy, radiotherapy, orthopaedic and cardiac treatments in the listed hospitals based on the level of cover you have chosen. You also have full cover for most Irish medical consultants as the majority of them are fully participating with Aviva.

On your 'we plan' Level 5 you're fully covered for hospital accommodation costs, consultant fees, diagnostic tests for cardiac care as well as procedures for treating heart conditions - including cardiac surgery, heart bypass, heart transplant, angioplasty, catheterisation, stenting, valve surgery and pacemakers. This includes full cover in high-tech hospitals for listed cardiac procedures. Following selected cardiac procedures, to help you recover, we will supply you with a home help assistant. We will contribute €100 a day to the cost of your home help for up to 3 days following a selected cardiac procedure. Your home help assistant will be supplied by an approved provider. |

Cancer Support Benefit This benefit is payable to members for accommodation costs incurred when travelling to receive in patient or day case chemotherapy or radiotherapy. Aviva will contribute up to a maximum of €1500 per calendar year. Dated eligible receipts on headed paper will be necessary to avail of this benefit. The member must be the person receiving chemotherapy or radiotherapy, and must have travelled more than 50 Kilometres, each way, from home in order to receive treatment. |

If you're covered for a medically necessary surgical procedure in Ireland but for whatever reason want to have it carried out abroad...you can. Your 'we plan' level 5 covers you and your family for out patient costs. These are medical expenses that you pay for yourself when attending your consultant or hospital as an out patient when you're not actually undergoing a stay in hospital. They include cover for out patient consultant visits, radiology and pathology procedures.
*U Mamma therapies and classes qualify for the pre and post natal benefit scheme. Remember to keep all of your out patient receipts and then claim at the end of your policy year. When you make the claim, we'll deduct the €200 excess and pay you the rest without delay. Here is an example of how to calculate your excess. |

Having discussed the treatments available to cancer sufferers with health care professionals we've identified another benefit we can offer. In recognition of the stress cancer treatment can cause, Aviva will now contribute €40 per visit towards the cost of 5 visits to a psycho-oncologist following chemotherapy to assist recovery. This benefit will be available under our out patient benefits. As such it will be subject to the out patient excess. The sessions must be conducted by a psychologist who is either a member of the Irish Association of Counselling and Psychotherapy or the Psychological Society of Ireland. |
