Before treatment, if you are in doubt about your cover, please call our customer service team on 1850 717 717. Remember to have your hospital name, consultant name and procedure code to hand.
If you are claiming for a hospital stay, simply complete the Aviva Health Claim form in hospital. We have direct payment agreements with our listed hospitals and can settle the bill directly for all eligible costs under your plan.
If you are claiming for an outpatient or day to day expense, keep all your receipts throughout the year. Your receipt should include your name, date of treatment and confirmation of your practitioner.
Send your receipts at the end of your policy year, with your name and membership number to: Claims Team, Aviva Health Insurance Ltd.,
